The requirements of regulatory legal acts of Ministry of Education and Scence of Ukraine and the Regulations on the procedure for the production, issuance, registration and sample forms of documents on higher education (academic degrees) and their appendix, a sample academic transcript at Odesa State University of Internal Affairs for obtaining duplicate documents on higher education (diploma of higher education and/or supplement to the diploma of higher education) state the following:

 

Duplicate documents on higher education are made and issued by the university upon written application of the person in whose name the document on higher education was issued, submitted by him/her personally or through an authorized representative to the university, which is registered in accordance with the law.

 

The application for issuance of a duplicate document on higher education shall include surname, first name and patronymic (if any), date of birth; series (if any), number, by whom and when the document certifying the person and confirming the citizenship of Ukraine or the document certifying and confirming his/her special status was issued; registration number of the taxpayer's account card (if any); unique number of the entry in the Unified State Demographic Register (if any); place of residence, telephone number of the person in whose name the higher education document was issued; name of the higher education institution (its separate structural subdivision) and year of graduation; name of the higher education document, a duplicate of which is ordered; reason for ordering a duplicate (loss; theft of the higher education document; damage to the higher education document that led to a violation of the integrity of the information; if, after entering information on the issuance of the higher education document into USEDE, errors in the information reproduced in the higher education document are found; other information that the person in whose name the higher education document was issued considers essential for obtaining a duplicate).

 

The application must be accompanied by copies of the following documents: a copy of the registration number of the taxpayer's account card (if any); a copy of the foreign passport (if any) for writing the surname and name in English; a copy of the passport of a citizen of Ukraine; a copy of the diploma and its supplement (if any), a copy of the marriage certificate (in case of change of surname).

 

Duplicate documents on higher education are issued on the basis of an order of the rector of the university within twenty calendar days and are certified in the register of issued documents on higher education by the signatures of both the person who issued such a document and the person who received it.
