Preparation programs for entrance exams

Requirements for writing a motivation letter

List of documents for candidates for higher education institutions of the Ministry of Internal Affairs of Ukraine required for medical examination in the Medical (military medical) commission.

Candidates from among civilian youth must have with them when passing the Medical (military medical) commission.

a referral from the personnel office (valid for up to 30 days; indicate the number, date of issuance of the referral, name of the MIA educational institution, faculty, contact phone number of the person who issued the referral);

registration certificate or military ID;

passport or birth certificate;

outpatient card from the place of residence. After passing the commission, the card is returned to the medical institution.

The following examinations are to be carried out at the place of residence:

Tests: (valid for 1 month):

complete blood count (with leukoformula)

complete urinalysis (with sediment microscopy)

blood sugar test;

serological blood test for (antibodies to human immunodeficiency virus (HIV), antigen to hepatitis B virus (HBsAg), antibodies to hepatitis C virus (anti-HCV);

blood test for RV;

blood group and Rh affiliation;

chest x-ray;

ECG (tape with description).

Certificates:

certificate and certificate from a psychiatric dispensary;

certificate and certificate from a narcological dispensary;

certificate from a tuberculosis dispensary (if necessary);

Certificate of preventive vaccinations, filled in according to the form 063/о, approved by the Order of the Ministry of Health of Ukraine No. 1 dated 10. 01.2006, No. 1;

certificate from the polyclinic at the place of residence (an extract from the outpatient card for which diseases, injuries were treated and registered for at least 5 years);

photo 3×4 cm.

All medical documents must be certified by the seal of the medical institution and the personal seal of the doctor.
